Our Geriatric Assessment & Wellness (GAW) Program offers comprehensive, personalized support for older adults and their families whether you're navigating new care needs, transitioning to retirement living, or simply seeking ways to age with resilience and purpose.
Our team consisting of a Social Worker, Occupational Therapist, and Registered Nurse - provides integrated support in mental health, home safety, and aging-in-place planning.

We begin with a complimentary 15-minute phone consultation to determine the suitability of our program and recommend the right level of care.

Level Two: TRANSITIONAL
CARE NAVIGATOR
Approx. 5-7 hours over 2-3 weeks | $879.75 + HST • Includes
Level Four: COMPREHENSIVE
CASE MANAGEMENT
Approx. 14-15 hours over 3 months | $2,111.40 + HST
Level Five: EXTENDED FAMILY CARE PLAN
15+ hours over 3-6 months | $2,932.50 + HST Includes Level 4 services
Receipts provided for private insurance reimbursement. Each phase is carefully followed to ensure safety, integration, and long-term results.
